Free consultations are not intended to diagnose, treat, or create a formal plan of care. It does not constitute a provider-patient relationship. General questions may be limited to cost, types of medications prescribed, or type of conditions treated.  A free consultation may last 10-15 minutes.

This will include the completion of screening forms prior to your appointment, obtaining a history, creating the provider-patient relationship, and may include prescribing medication.

The is for the established mental health patient with a current plan of care in place. Screening forms may need to be completed prior to your appointment and may include adjusting medication or refills if you are currently prescribed medication. 

Although chronic conditions may not be managed long-term with LOSA. LOSA does provide 30 day or longer refills giving you time to get back to your primary care provider for further management. The option to provide acute illness visits fit most people's busy life; increasing access to care on the go.

Many facilities now require people to complete work excuse forms for any absence. Cost can be discussed based on the number of documents to be reviewed and completed.
